Remove all unnecessary fields from each account type in the My People area in Monarch.

There are many fields in all account types that aren't needed and are confusing at to why they're included.

Example:

There are 34 fields in the student account (exlcuding the clinical fields for the practitioners). We only fill out about 10 fields when we have all the info required.
