Small Update to Referral Tool - Diagnosis

A customer shared with me (they were using an older version of our PDF intake form) that we ask for a diagnosis but that in the Online version it doesn't ask for what that diagnosis is. There is a text box where those notes could be added, but it doesn't prompt sharing that information and they weren't sure it was appropriate to put there. (It states "What influenced this referral?")

Could we either update the text box to include a prompt like " What influenced this referral, and please share any additional information, such as diagnosis if one applies" OR if they select "yes" to diagnosis, a text box appears for them to enter what it is? Our practitioners often as a followup will ask what it is, sometimes before accepting a referral. This would save back and forth communication and streamline referrals. I'm not sure why we ask yes or no to a diagnosis if we don't ask them to clarify what it is - I think this prompts more valuable information upfront.
