Provide Deactivation reason of Duplicate account

Often we accidentally create duplicate school or eHelper or student accounts and Monarch does not catch this. One the error is made, the only option is to deactivate the account. To allow quicker clean up an merge, if when selecting deactivate we could have the option of Duplicate account and a field to enter the account number it should be merged with.
